Community organizations and health care providers across Fond du Lac County are working together to make it easier for people to access the services they need. Through the Unite Us platform, organizations can improve communication, coordinate referrals, and help connect people with resources that address needs beyond medical care. This can also help providers better understand what resources a person has already received and identify additional services that may be needed.
For people facing challenges such as homelessness, food insecurity, housing instability, or other barriers, this can make a meaningful difference. Rather than navigating multiple systems on their own, people can have a clearer path to available resources and services.
This approach is an important part of strengthening integrated care — recognizing that the factors affecting a person’s health often extend beyond what happens in a medical setting. Access to stable housing, nutritious food, transportation, family resources, and other basic needs can all play a role in a person’s ability to access and benefit from care.
ADVOCAP is proud to be part of this initiative alongside community partners throughout Fond du Lac County. Through programs focused on basic needs, housing stability, childhood development, workforce opportunities, and supporting older adults, ADVOCAP connects our neighbors with a wide range of resources.
The initiative brings together organizations including ADVOCAP, Charis Pregnancy and Family Help Center, St. Katherine Drexel Shelter, Solutions Center, Holy Family Warming Shelter, Fondy Food Pantry, Fond du Lac County Health Department’s Save A Smile Dental Program, and SSM Health’s Samaritan Clinic and Case Management teams.
By bringing health care and community organizations together through Unite Us, this initiative helps build stronger systems of integrated care that extend beyond hospital walls and connect our neighbors with the resources they need.
