We are a private, nonprofit Community Action Agency committed to our mission of “creating opportunities for people and communities to reduce poverty and increase self-sufficiency.”

ADVOCAP’s Homeless Prevention program is accepting applications for a Homeless Prevention Specialist for our Berlin office. Our Homeless Prevention program is in Fond du Lac, Green Lake, and Winnebago counties. We assist individuals and families to overcome homelessness and/or potential homelessness and poverty by establishing and implementing an individual plan for them to achieve self-sufficiency, especially housing stability.

RESPONSIBILITIES INCLUDE:
  • Respond to individuals and families in crisis and assist them in addressing basic needs
  • Work with individuals and families in establishing their plan for self-sufficiency
  • Coach participants along their path to self-sufficiency
  • Connect participants to appropriate community resources and document the individual’s needs, plans, services provided, and outcomes achieved
  • Identify gaps in community services
  • Maintain records documenting participant progress
  • Assess and enroll participants into appropriate programs
  • Establish trusting relationship with people
JOB REQUIREMENTS:
  • This position will have a bachelor’s degree in human services or social services, and 2 years of previous case management work experience or an equivalent combination is required to meet job standards
  • Possess an understanding of poverty and the ability to be non-judgmental with those served
  • Ability to develop positive, growth-directed relationships with individuals and families in poverty
  • Knowledge of and experience in utilizing community resources to help people in need
  • Acceptable drug screen and comprehensive background check required
  • Reliable attendance and a professional work ethic
SALARY & BENEFITS

Full-time, 36-hour workweek position with an entry hourly wage of $22, based on experience. ADVOCAP has an excellent benefit package. This full-time position will have access to health, dental, vision, life, short- and long-term disability, vacation, sick and holiday pay, and our employer-contribution 401k.
